The APM FPT Work Group met on July 31, 2015 to discuss their proposed APM framework categories and methods for tracking and measuring the progress of APM adoption across health care systems. Dr. Rahul Rajkumar from the Centers for Medicare & Medicaid Services (CMS) presented the APM categories to the members, noting that the categories were created to establish a common APM language among stakeholders. The members gained consensus on the essential elements of the APM framework, which include but are not limited to educating patients and consumers about their individual roles and responsibilities within categorized APMs, and emphasizing stakeholder alignment with costs and quality metrics.

To achieve the goal of better care, smarter spending, and healthier people, the U.S. health care system must substantially reform its payment structure to incentivize quality, health outcomes, and value over volume. The Health Care Payment Learning and Action Network (LAN) was established as a collaborative network of public and private stakeholders, including health plans, providers, patients, employers, consumers, states, federal agencies, and other partners within the health care community.
